Supervised Practice Agreement & Evaluation

The supervised practice period involves an evaluative component which both the supervisor and supervisee are expected to participate in.

Both the supervisor and supervisee must:

1. Review and sign supervision agreement.

• • A signed supervision agreement must be received by ACSLPA before a conditional permit can be issued.

2. Complete midterm evaluation by given deadline.
3. Complete final evaluation by given deadline.
