SUMMARY OF DISCIPLINARY COMPLAINT RESOLUTION AGREEMENT

The Regulated Member and the College entered into a Disciplinary Complaint Resolution Agreement in December 2025.
The Regulated Member agreed that in or around May 2025 – July 2025, he engaged in unprofessional conduct in that:
a. On three occasions, the Regulated member failed to ensure the accuracy of assessment and diagnosis information in assessment reports.
b. On one occasion, the Regulated Member failed to adhere to standardized assessment protocols for a child client while being directly observed.
The conduct admitted to in the DCRA arises from one complaint. The Regulated Member agreed to complete education and provide a self-reflective paper with a focus on assessments and reports (including clinical reasoning and professional judgment). The Regulated Member must also complete a period of six (6) months of monitoring in his workplace related to assessments as a SLP.
